We have created a set of PDF worksheets to help you write your entries. (You can also email us any questions you might have at fast50@fastcompany.com .) To download the PDF guides, you'll need the Acrobat reader from Adobe.

Click on the link below to download the appropriate file. You might want to save it on your desktop. To do so, right-click the link below (control-click on a Mac), and choose "Save target as ..." (choose "Save link to disk" on a Mac) to save the PDF to your hard drive. Once the download is complete, you can double-click on the file, and it will open through Adobe Acrobat. Once you print out the guide, don't forget to return to fastcompany.com to enter your story!
